<html><p align='center'><strong>Physical Therapist</strong></p> <hr /> <p>Provides and directs Physical Therapy services to residents to include assessment and treatment, development and implementation of programs, and related professional in-services. Performs duties under the direction of physician?s orders.</p> <p> </p> <p><strong>Responsibilities:</strong></p> <ul> <li>Provides Physical Therapy services to residents and initiates referrals when appropriate and in accordance with Physical Therapy Department procedures. <li>Conduct screenings of residents at regular intervals to determine need for intervention/treatment. <li>Evaluates residents to obtain data necessary for treatment planning and implementation. Conducts specialized evaluations as indicated. <li>Interprets and communicates evaluation findings to residents, family, and other team members. <li>Monitors resident?s response to intervention and modifies treatment as indicated to attain goals in accordance with physician?s orders. <li>Participates in discharge planning including programming to maintain and enhance the performance of the resident in his or her own environment. <li>Documents results of resident?s assessment, treatment, follow-up, and termination of services. <li>Reviews the quality and appropriateness of the total services delivered and of individual Physical Therapy programs for effectiveness and efficiency, using predetermined criteria. <li>Supervises Rehabilitation Aides, students, volunteers, directs LPTAs in the implementation of the treatment plan <li>Assumes responsibility for ongoing continuing education and professional development <li>Complies with all Infection Control, Universal Precautions and OSHA standards for the health care professional.
